ok vi parlerò della della qualità delle informazioni su internet e in
particolare anche nellambito dei siti della fibrosi cistica sulla
fibrosi cistica 0:41 //
e in particolare vi mostrerò prima dei dati da alcuni sondaggi
importanti internazionali sullutilizzo di internet negli anni e anche
poi sullutilizzo di internet per la salute 0:55 //
poi vi parlerò brevemente di una dei risultati di una revisione ancora
nello stato iniziale che abbiamo condotto sulla qualità cosiddetta
qualità degli s- dei siti italiani sulla fibrosi cistica 1:11 //
e poi farò delle conclusioni finali sul lutilizzo di internet 1:15 //
comunque sarò breve non credo di parlare per unora 1:18 //
ormai si sa che internet influenza tutti gli aspetti della vita da gli
acquisti i banali acquisti ai viaggi al controllo della posta
elettronica al meteo e ovviamente anche alla salute 1:36 //
e la lutilizzo continua ad aumentare come mostrano i sondaggi 1:41 //
da quando ha iniziato a prendere a diventare una uno strumento
importante e ad entrare nelle case delle persone dal circa
novantacinque è aumentato enormemente luz- lutilizzo di internet 1:54 //
da ques- da questa tabella si può vedere che negli anni in particolare
dal duemila al duemila e cinque cè stato un aumento notevole in vari
paesi 2:6 //
nel Nord America per esempio cè stato il minor aumento solo del centodieci per cento 2:13 //
però comunque si può vedere che internet è arrivato nel </nal/> al a far parte del settanta per cento della popolazione 2:21 // ossia il settanta per cento della popolazione americana utilizza internet 2:25 //
in altri paesi per esempio nellAsia cè solo il dieci per cento della popolazione che utilizza internet 2:31 //
però se si va a vedere poi la percentuale della popolazione dellAsia
rispetto alla percentuale mondiale i numeri insomma prendono più
significatività 2:42 //
si può vedere anche che negli ultimi cinque o sei anni cè stato un
aumento molto più grosso in alcune aree del mondo che sono quelle che
iniziano adesso ad avere accesso a più a strumenti di informazioni come
internet 2:58 //
per esempio si vede che in Africa cè stato un aumento in questi ultimi cinque anni del più del quattrocento per cento 3:4 //
in Europa giusto per sottolineare abbiamo quasi il duecento per cento di aumento 3:10 //
vi presenterò ora i alcuni concetti principali che risultano da un sondaggio della HON nel nellultimo periodo 3:24 //
la HON è unorganizzazione svizzera significa Health On the Net ossia salute su internet 3:32 //
ed è unorganizzazione che tra quelle che più si adopera per la qualità su internet 3:37 //
hanno questa organizzazione anche è diventata famosa perché ha creato
un sistema per tentare di certificare la qualità di dei siti sulla
salute su internet 3:50 //
questa organizzazione ha anche condotto questi sondaggi dal
novantacinque ha raccolto informazioni su moltissimi utenti su internet
4:1 //
questi sono i risultati dellultimo di uno degli ultimi sondaggi 4:5 // in particolare è stato fatto alla fine del duemilaquattro fino all'inizio del duemilacinque 4:9 //
copre circa quattro mesi se mi ricordo bene e ha compreso circa duemila
utenti che sono andati sul sito della HON o su altri siti dove sono
stati esposti al questionario e hanno risposto in circa duemila 4:25 //
gli utenti provengono per circa la metà dagli Stati Uniti e circa un
quarto dallEuropa e per il resto coprono vari vari paesi del mondo 4:37 //
si è visto che gli utenti cercavano informazioni sulla salute in media tre o più volte alla settimana in generale 4:44 //
e ancora più gli europei degli americani che è un dato un po sorprendente 4:49 //
tra i tipi di utenti che hanno risposto cera il quarantacinque per
cento di utenti generali o pazienti e il cinquantacinque di
professionisti tra i quali cerano principalmente medici di medicina
generale professionisti specialisti e infermieri 5:8 //
cosa è risultato da questo sondaggio 5:13
// beh rispetto al al rapporto con il medico si è visto che la metà di
di coloro che usano internet hanno cercato informazioni sulla salute in
seguito a una conversazione col medio 5:26 //
mentre la metà discute le informazioni che trova su internet con il medico 5:31 //
e poi il settanta per cento in generale cerca informazioni per supplementare quello che gli viene detto dal medico 5:38 //
questi dati mostrano comunque una un buon rapporto con internet e il medico 5:45 //
e un suggerimento dalla parte degli utenti che è saltato fuori dal
sondaggio che molti suggeriscono che il medico oltre a prescrivere
farmaci dovrebbe anche prescrivere dei siti che ritiene affidabili
ovviamente sulla qualità 6:1 //
riguardo al rapporto specifico paziente-medico si è visto che circa la
metà dei pazienti ritiene che consultare internet favorisca una
relazione più informata col medico 6:15 //
l'al- l'altra parte dalla parte del medico la stessa cosa risulta //
la quasi totalità dei professionisti esprime lo stesso giudizio 6:24
// ossia pensa che lutilizzo di internet dalla parte dellutente possa
aiutare o favorire la comunicazione linterazione con il medico 6:33 //
alcuni dicono anche alcuni che hanno fatto degli studi hanno trovato
che lutilizzo di internet possa anche ridurre il tempo che serve
durante una consultazione medica per capire per scambiare informazioni
tra il medico e il paziente 6:49 //
cè anche poi il fatto che un terzo dei professionisti giudica utile
per la gestione delle cure dei propri pazienti che questi consultino
siti affidabili 7:1 //
riguardo alla consulenza on-line sono stati raccolti anche per questo
aspetto dei dati e si è visto che sono abbastanza utilizzati 7:13 //
il quaranta per cento degli americani e il quarantotto per cento degli
europei u- utilizza almeno qualche volta questi tipi di servizi 7:22 //
e com- sono utilizzati anche abbastanza frequentemente perché siamo sempre al quaranta per cento circa delle persone 7:31 //
ci possono essere degli aspetti positivi e negativi ovviamente con questo utilizzo di internet 7:40 //
dalla parte del paziente può essere visto più positivamente il fatto
che cercare su internet possa aumentare linteresse dellautogestione
della malattia dalla parte del paziente 7:54 //
dall'altra parte invece dalla parte del professionista circa il
settanta per cento dice che lutilizzo la ricerca di informazioni
sanitarie su internet possa aumentare il rischio di autogestione 8:8 //
dipende dal punto di vista e dal tipo di autogestione ovviamente 8:14 //
si è visto anche che gli utenti che utilizzano internet per la salute
utilizzano o tengono conto di diversi tipi per cercare di valutare la
qualità dei siti 8:29 //
si è visto in particolare che molti tengono presente i sistemi di certificazione o accreditamento //
e il più signi- il più conosciuto è il HON code che è quello che ho menzionato prima 8:40 // molti
co- utilizzano anche un altro sistema // ossia filtrano i risultati i
siti che risultano dalle loro ricerche in base al tipo di sito 8:51 //
questo è un discorso più valido per i siti americani perché utilizzano più questi tipi di domini 8:60 //
ossia i più frequenti sono sono quelli i siti di tipo governativo o che
hanno a che fare con il sistema scolastico educativo le università e
così via 9:13 //
tra i tipi di siti ritenuti meno credibili dalla parte degli utenti
erano cerano i punto com o quelli associati in qualche modo a un
aspetto commerciale 9:26 //
riguardo alla qualità in internet in generale si può dire perciò che
lutente mostra ha mostrato in questi anni un aumento generale nelluso
dei sistemi per distinguere i siti affidabili sulla salute 9:42 // e questo mostra un aumento della consapevolezza dell'importanza di essere critici 9:48 //
qualche anno fa era più probabile che l'utente generale comune andasse su internet e si fidasse di quasi tutto ciò che trovava 9:59 //
adesso invece vi è si si ritrova un un un un aspetto più critico da parte dellutente 10:9 //
questo brevemente è un altro sondaggio // è un sondaggio americano sullutilizzo di internet per la salute 10:18 //
i dati vengono dal provengono dagli anni duemila e due duemila e tre //
e questo sondaggio che è stato spedito a dodicimila persone e che ha
avuto circa ottomilacinquecento risposte dovrebbe essere
rappresentativo della popolazione americana 10:34 //
e questo sondaggio contemplava anche il la presenza di malattie
croniche e il tipo di rapporto dellutente con internet in base alla
presenza o meno di queste malattie croniche 10:47 //
in generale si è visto che il trentacinque per cento degli utenti cercava informazioni su internet sulla salute su internet 10:55 //
dove cercavano 10:56 //
cercavano principalmente dai portali di informazioni che sono dei siti
che danno collegano o guidano lutente a varie fonti diverse sulla
salute 11:9 //
s- successivamente in secondo ordine di importanza ci sono i siti governativi come dicevo prima 11:19 //
e poi i siti delle organizzazioni no profit 11:22 //
sul motivo perché gli utenti cercavano informazioni sulla salute su
internet cerano il fatto che fosse facile ricercare che le
informazioni fossono- fossero chiare e semplici e che fossero gratis 11:40 //
e poi che alcuni ritenevano che le fonti erano affidabili 11:45 //
si è visto poi che in questo sondaggio che circa il cinquantasette per cento aveva una o più malattie croniche 11:56 //
questo dato in realtà probabilmente è una sottostima 11:59
// perché nel sondaggio avevano hanno contemplato solo un elenco di
malattie croniche tra le quali non cera la fibrosi cistica ma cera il
diabete la depressione e altre malattie 12:13 //
e si è visto comunque che tra queste persone che hanno s- segnalato di
avere una o più malattie croniche si è visto che tra queste persone e
le persone senza malattie croniche non cera molto molta differenza sul
tipo di sito frequentato 12:30 //
se non per un aumento dellutilizzo dei siti o dei servizi tipo chat
room di discussione on-line dalla parte delle persone con malattie
croniche 12:42 //
questo da alcuni studi si è visto che può essere perché le persone
trovano più supporto nel avere un modo di comunicare con altre persone
nella stessa situazione con la stessa patologia 12:57 //
prima di andare avanti volevo solo brevemente dimostra- mostrare parlare di della qualità 13:10 //
perché ci sono diversi concetti di qualità 13:14 // vari studi sono stati condotti sulla qualità dei siti però non sempre questi studi valutavano lo stesso tipo di qualità 13:23 //
ossia alcuni gruppi studi comunque valutano la qualità rispetto a
laderenza a determinati criteri estetici cosiddetti estetici 13:36 //
tra questi possono essere la presenza o meno del nome dellautore delle
informazioni della della presenza o meno delle referenze da dove
vengono quelle informazioni e così via 13:48 //
poi altri studi o altri gruppi hanno valutato la qualità dei siti rispetto alla completezza 13:54 //
per esempio hanno scelto alcuni argomenti o alcune malattie e hanno
individuato degli aspetti di quelle malattie che il sito secondo loro
doveva trattare per essere completo 14:5 //
però con un sistema uno strumento come internet è quasi impossibile
valutare la completezza valutare la qualità in base alla completezza
perché ci sono talmente tanti siti che [#] va a cercare informazioni su
più siti 14:22 //
e non è è anche de- non è detto che siti diversi abbiano lo stesso tipo di utente come destinatario principale 14:30 //
ci sono siti che vogliono dare informazioni solo solo su un aspetto di
una malattia e altri che vogliono dare informazioni generali senza
entrare troppo a fondo di una malattia 14:40 //
il terzo concetto di qualità è la qualità rispetto alle linee guida o alle conoscenze scientifiche 14:47 //
questo è lunico modo per valutare veramente la qualità del contenuto di un sito 14:53 //
vuol dire l- guardare le informazioni sulla malattia sulla terapia
anche di quel sito e confrontarle su quelle che sono le conoscenze
attuali cliniche 15:5 //
ci sono diverse son state create diverse strategie o iniziative negli
ultimi anni per cercare di trovare un modo per valutare la qualità dei
siti 15:21 // anche qui però con diversi approcci 15:24 //
ci sono per esempio i codici di condotta 15:29 //
in questo caso un esempio conosciuto sono le linee guida dell'associazione americana medica 15:37 //
ed è un codice di condotta che dice che chi crea i siti sulla salute dovrebbe seguire alcuni criteri 15:46 //
poi ci sono i sistemi che hanno cercato di valutare la qualità in base alla certificazione da terzi 15:56 //
ossia il famoso HON code che significa che un altro gruppo un gruppo
esterno un terzo valuta un sito in base a determinati criteri e gli
applica gli gli accredita un certo una certa riconoscenza 16:11 //
e poi ci sono le guide specifiche per l'utente // in questo caso un esempio è Discern 16:18 //
queste guide forniscono allutente un elenco di criteri che lutente
può applicare a un singolo sito semplicemente rispondendo a queste ad
alcune domande per vedere se è probabile che quel sito sia di qualità 16:35 //
di nuovo // questo lo dico successivamente 16:39 //
sono stati fatti è stato fatto uno studio il più grande che ha cercato
di raggruppare tutte le varie tipi </tipe/> di tipi di iniziative
fatte negli anni e ha trovato addirittura ventinove iniziative diverse 16:54 //
ha raggruppato i criteri e i principi di queste varie strategie queste
varie iniziative e ha visto che alla fine la maggior parte erano
sovrapponibili 17:5 //
cioè tutte le varie i vari tipi di strategie andavano alla fine a guardare le stesse tipi di informazioni 17:12 //
questo solo brevemente questo è lesempio del codice di condotta che ho menzionato prima dell'associazione medica americana 17:24 //
e questo codice di condotta per chi crea i siti dice che sulla salute dice che i siti dovrebbero seguire tutti questi criteri //
ossia che ci sia il nome dellautore che ci siano le referenze indicate
sul sito che ci s- sia dichiarato i vari i finanziamenti che ci sia un
certo revisio- una certa revisione del materiale da esperti 17:50 //
bisogna dichiarare il metodo di revisione e fornire la data di creazione o di aggiornamento del sito e così via 17:59 //
invece per il second- la s- il secondo la seconda strategia ossia la
certificazione da terzi il famoso HON code questi sono i loro principi 18:10 //
cioè un c- un sito per poter utilizzare o mostrare il codice il
bollettino blu il bollino blu scusate della HON code quel sito deve
aderire a questi criteri // e s- vedete che sono praticamente gli
stessi 18:27 //
deve essere presente il nome dellautore le sue credenziali le referenze la data di aggiornamento e così via 18:34 //
in particolare questo segnala che deve essere presente la frase che
linformazione fornita sul sito non deve sostituire le cure del medico 18:43 //
questo è il terzo tipo di strategia 18:47 //
ossia la guida per lutente 18:49 //
e di nuovo abbiamo le referenze il nome dellautore chi è responsabile dellinformazione insomma 18:57 //
però questo va fa un passo oltre 19:1 //
chiede allutente che teoricamente non sa valutare se le informazioni
del sito sono buone o no chiede allutente anche di dare un parere per
esempio su s- sul trattamento specifico o sulla malattia un parere più
specifico 19:22 //
per esempio chiede allutente se il sito secondo lutente dà informazioni di parte 19:29 //
per esempio se dà informazioni soggettive o se sembrano oggettive 19:34 //
se cita più fonti di informazioni 19:37 //
se parla di aree di insicurezza nel trattamento cioè parla di diverse
opinioni di esperti oppure se dice che un trattamento è al cento per
cento efficace in tutta la popolazione pertinente 19:51 //
poi per esempio anche in dettaglio se il sito spiega cosa succederebbe se non fosse trattata la malattia e così via 20:1 //
questo per darvi un'i- unidea di dei vari criteri che vengono utilizzati da questi sistemi 20:8 //
i problemi con i vari metodi quali sono 20:12 //
beh tutti innanzitutto sono collegati al fatto che internet non ha una struttura fissa 20:18 //
è è in continuo cambiamento 20:20 //
e in più come dicevo prima manca un audience definito 20:27 //
un sito può essere per un professionista può essere per un un bambino può essere insomma varia 20:34 //
e perciò per i codici di condotta per esempio questi codici non sono
obbligatori e possono essere interpretati in maniera diversa 20:43 //
riguardo alla certificazione da terzi il bollino blu può essere applicato quando il sito viene revisionato o rivisto dal terzo 20:53 // però poi il contenuto del sito può cambiare e il bollino blu rimane finché non viene rivalutato il sito 20:59 //
rispetto alle linee guida per lutente queste si basano sul principio
che lutente abbia il tempo e lattitudine latteggiamento ad applicare
lo strumento in maniera corretta 21:10 // e la capacità di applicare lo strumento in maniera corretta 21:14 //
in più posso dirvi che sono stati fatti molti studi per valutare la
capacità di queste varie strategie per cercare di misurare la la vera
qualità delle informazioni nel sito e questi studi hanno dimostrato che
queste strategie non sempre sono applicabili 21:37 //
nel senso che uno studio per esempio ha valutato la presenza o meno del
bollino blu il HON code e ha visto che la qualità in alcuni siti era
buona e in altri no 21:48 //
e non importava se dimostravano se mostravano il bollino blu o meno 21:53 //
come anche per singoli criteri o se la presenza del nome dellautore o le referenze 22:0 //
non è detto che i siti che danno queste informazioni siano diano informazioni corrette accurate 22:7 //
cè anche da tener conto del livello di linguaggio utilizzato nei siti 22:15 //
uno studio del duemilauno per esempio ha valutato le informazioni per
quattro patologie e ha visto che il livello del linguaggio utilizzato
era delle superiori o del decimo anno scolastico che è più alto della
media della popolazione 22:31 //
questo è stato valutato sia per pagine in inglese che in spagnolo 22:36 //
uno studio del novantanove la stessa cosa 22:40 // ha valutato dei siti sulla salute in inglese e ha visto di nuovo che era il linguaggio era di un livello delle superiori 22:49 //
rispetto ai danni subiti da internet cè stata molta diciamo ci sono state molte avvertenze ma pochi s- fatti documentati 23:3 //
sicuramente ci sono stati dei danni ci- ancora oggi tutt'oggi 23:8 //
però rispetto a tutta la la la la media il messaggio dei media di stare
molto attenti a internet insomma non cè poi proprio tanto riscontro
nella nella documentazione scientifica 23:20 //
è stata fatta una revisione nel duemila e due che ha cercato tutti gli
articoli scientifici su possibili danni su millecinquecento che
potevano insomma che affrontavano largomento ha trovato solo tre casi
di danno specifici 23:39 //
uno riguardava danni emotivi alla persona in base come risultato del
dellaver consultato informazioni poco appropriate su internet 23:51 //
uno er- era di danni al ai cani di un utente 23:55 //
e uno era in effetti un caso di danno effettivo in una persona con
carcinoma che si è rivolto al medico dopo un anno di automedicazione a
casa seguendo un delle informazioni trovate su Internet 24:11 //
è stato fatto anche un questionario a dei medici inglesi nel duemilauno 24:16 //
e questi hanno segnalato che tra i loro pazienti era capitato una per
esempio una morte da sovradosaggio di un farmaco </farmico/>
acquistato su Internet 24:27 //
e due casi fatali di tumore presentati tardi dopo di nuovo l'automedicazione 24:33 //
studi sulla qualità 24:38 //
ne sono stati fatti moltissimi e in tutte le aree tutte le popolazioni tutte le età 24:44 //
questi variano dalla tosse alla febbre che abbiamo condotto allistituto Mario Negri 24:51 //
forse è anche per questo che ci hanno chiesto di parlare qui oggi 24:54 //
ci sono anche stati fatti sulla depressione sul diabete fibrosi cistica incontinenza urinaria e così via 25:1 //
una revisione che ha messo insieme le conclusioni di tutti questi studi
ha trovato che il settanta per cento di queste conclusioni trovava una
qualità scarsa di informazioni su internet 25:16 //
però come dicevo prima molti di questi studi non valutavano la qualità
vera delle informazioni ma valutavano quegli aspetti estetici o di
completezza 25:30 //
ci sono diversi studi anche sulla qualità delle malattie croniche 25:36 // depressione malattie renale diabete e così via 25:40 //
e questi hanno trovato dati insomma non concordanti 25:48 //
tra quelli che hanno valutato in base il contenuto dei siti in base
alle linee guida o insomma in base alle conoscenze scientifiche hanno
trovato nel caso del diabete per esempio che l'accuratezza
</accuratezia/> variava 26:2 //
nel caso dellincontinenza urinaria hanno trovato che i siti davano informazioni abbastanza accurate 26:8 //
mentre alcuni altri studi hanno mostrato poca concordanza con 26:14 //
no questo è un altro 26:16 // hanno poca concordanza con i criteri definiti dal codice HON che dicevo prima 26:23 //
uno studio è stato fatto sulla qualità dell'informazione sulla fibrosi cistica del nel duemila e due 26:31 //
questo studio però non ha valutato specificamente la qualità ma ha
valutato laderenza del sito al codice di condotta dellassociazione
americana 26:44 //
e ha trovato che cera poca concordanza 26:48 //
che meno ella metà dei siti forniva per esempio la data di aggiornamento del sito 26:55 //
che quaranta per cento dava la fonte dellautore o delle informaz-
delle informazioni e che ancora meno specificava le informazioni
specificava che le informazioni non dovevano sostituire le cure del
medico 27:9 //
perciò in generale ha trovato poca aderenza al codice di condotta 27:13 //
adesso vi presento brevemente i risultati di una revisione che abbiamo fatto recentemente //
stiamo ancora valutando analizzando i risultati 27:23
// abbiamo deciso di fare questa revisione sui siti italiani sulla
fibrosi cistica per vedere la qualità e anche per fare un passo
successivo allo studio che ho citato prima sulla fibrosi cistica 27:37
// perché quel sito non valutava specificamente le informazioni ma
valutava laderenza al codice di condotta e in più valutava i siti in
inglese 27:46 //
noi abbiamo deciso di valutare quelli italiani 27:49 //
siamo partiti da un motore di ricerca generale // google 27:53 //
e abbiamo messo per la parte italiana la frase fibrosi cistica 27:59 //
e abbiamo trovato centosettantacinquemila pagine come risultato 28:3 //
di queste queste le abbiamo limitate solo alle pagine scritte in
italiano e solo a quelle pagine dove la f- la frase fibrosi cistica
fosse presente nel titolo 28:14 //
e il risultato abbiamo ottenuto quasi tredicimila pagine 28:20 //
abbiamo fatto la stessa cosa in inglese solo per avere un confronto con
il numero numeri di pagine e da quattordici milioni siamo arrivato
arrivati a quattrocentocinquemila pagine 28:32 //
una breve parentesi 28:35
// rispetto ai quattordici milioni di pagine fatte nella trovate
nellaprile duemila e sei nello studio precedente sulla fibrosi cistica
che ho citato prima con lo stesso criterio erano state trovate solo
centottantaseimila pagine 28:53 //
questo mostra negli ultimi quattro anni un aumento enorme delle pagine del numero di pagine sulla salute su internet 29:3 //
probabilmente però è anche a causa dellaumento di insomma dellottimizzazione dei motori di ricerca 29:10 //
continuando con la nostra revisione eravamo arrivati a
dodicimilanovecento pagine sulla fibrosi cistica o che in qualche modo
menzionassero la frase in italiano 29:21 //
abbiamo considerato le prime quaranta e abbiamo escluso quelle che non davano sufficienti informazioni sulla fibrosi cistica 29:30 //
e siamo abbiamo ottenuto trenta pagine 29:33
// queste trenta pagine sono state valutate da più persone in base a
laderenza a nove criteri estetici scelti da noi e in base anche
allaccuratezza dellinformazione sulla fibrosi cistica rispetto a
delle linee guida internazionali

so I'll be speaking to you about the quality of information on internet and particularly with regard to cystic fibrosis sites 0:41 //
and in particular Ill be showing you the data of some major
international surveys regarding internet use over the years and also
internet use for health-related topics 0:56 //
after which Ill be talking briefly about the results of a review which
is still ongoing which is still in its embryonic stages regarding the
so called quality of Italian cystic fibrosis sites 1:12 //
and then Ill come to my final conclusions regarding the use of internet but Ill try to be brief //
I don't think that I'll be overstepping the hour mark 1:21 //
we know that internet has a great deal of effects on all aspects of
life from purchases to travel to checking email checking the weather
forecasts and obviously among all this health 1:37 //
and the use of internet continues to grow as surveys have shown 1:43 //
since internet first became an important instrument and started to
become something available in many homes in the mid nineties the use of
internet has increased incri- has increased considerably 1:58 //
this table shows us that over the years and s- from two thousand to two
thousand and five there was a considerable increase in the number of
countries 2:6 //
in north America for example there was an enormous increase of a hundred and ten per cent 2:15 //
but internet is now something which is used by seventy per cent of the American population 2:24 //
in other countries for example in Asia only ten per cent of the population are internet users 2:31 //
but if we take a look then at the percentage of the population of Asia
by comparison with the world population I think that all this is
something that we can see better in context and in its true perspective

in the last five or six years there's been an increase which has been
quite major in some areas of the world which are the areas which are
now starting to have more widespread information resources like
internet 2:59 //
in Africa there has been an increase in the last five years of more than four hundred per cent 3:4 //
in Europe just to underline our situation we have or had almost two hundred per cent increase 3:12 //
Ill now be showing you some concepts some of the major concepts which
can be highlighted by a survey </survai/> of HON with regard to
the last few years 3:25 //
this is a Swiss organization 3:27 // and it means Health On the Net // H O N 3:31 //
and this is an organization which is one of the most active
organizations most committed to the question of quality on internet 3:39 //
and this organisation has become important because its created a
system in order to try to certify the quality of sites regarding health
and internet 3:49 //
and the organisation has run a survey focusing on a lot of users of internet involving more than two thousands users 4:3 //
and this is one of the most recent surveys at the end of two thousand and four and early two thousand and five //
it's a period of about four months 4:11 // here there were about two thousand users 4:15
// they access the HON site or other sites which were mentioned in a
questionnaire and they answered and about two thousand of them gave
responses to the questionnaire 4:28 //
about half are from the US about a quarter from Europe 4:32 // and the others covered various parts of the world 4:36 //
we saw that users were looking for information about health three or more times a week on average 4:44 //
and it was more Europeans than Americans concerned but this is rather surprising 4:49 //
in terms of the types of users who responded forty-five per cent were
general users or patients and then fifty-five per cent were
professionals including mainly GPs or specialists specialist doctors
and also nurses 5:8 //
and what emerged from this survey with regard to the relationship with the patient with the doctor 5:19 // we saw that half of those who use the internet look for information about health after a conversation with the doctor 5:26 //
whereas half of the users were discussing information found in internet with the doctor afterwards //
and seventy per cent generally were trying to complement information given by the doctor 5:37 //
these data show that there is a good relationship between internet and the doctor 5:45 //
and there is a suggestion from users that many suggest that the doctor
should not just prescribe drugs but also prescribe sites that he or she
thinks is thinks are reliable in terms of their quality 6:3 //
with regard to the specific patient-doctor relationship we saw that
about half of patients thought that consulting internet could favour a
more informed relationship with the doctor 6:15 //
then on the doctor's side the same was true 6:21 //
we have the same sort of judgement from almost all the professionals
who thought that the use of internet by users could favour
communication and interaction with the doctor 6:32 //
some also thought that the use of internet could reduce the time
necessary for a medical consultation in order to understand and
exchange information between the doctor and the patient 6:50 //
then we also have a third of the professionals
</opresprofessionals/> who think that its useful to use reliable
sites for the management of their patients 7:1 //
it's useful for the patients to consult these sites 7:4 // with regard to online consultation we also have some data in this respect 7:9 // and we can see that there is quite a lot of usage 7:14 //
forty-two per cent of Americans and forty-eight per cent of Europeans at least occasionally use this kind of service 7:22 //
on-line consultancy 7:24 // and this kind of service is used quite often 7:28 // about forty per cent of Americans about forty per cent of people overall use this kind of service quite often 7:34 //
there can be positive aspects and also drawbacks with this use of internet 7:39 //
with regard to the patient from the patients perspective it can be
seen as something positive that looking for things on the internet can
stimulate the interest in self management of illness by the patient 7:53 //
from the professionals perspective about seventy per cent of
professionals say that the research for health-based information on
internet can have increased risks involved in self management 8:8 //
so it all depends on what your point of view is and also it depends on the type of self management 8:12 //
we also saw that the users who consult internet on health-based topics
will use it and will take into account different considerations with
regard to the quality evaluation of the sites 8:30 //
many people will bear in mind the systems of certification or accreditation 8:35 //
the most well-known is the HON code that I mentioned before 8:39
// then others also use another system thats to say they filter the
results re- regarding their sites that they turn up on the basis of the
kind of site 8:51 //
and this refers to the American sites above all because they tend more to use these types of domino# suffix 8:60 //
or they use them more frequently 9:2
// this means that we have government sites or perhaps sites that have
something to do with the educational system or universities and so on
when they have e d u after them 9:13 //
among the types of sites that are thought to be less credible by the users we have the dot com sites 9:21 // or that's to say the ones which have some commercial overtones or associations 9:26 //
with regard to quality of internet what we can say generally is that
the user showed over the years a general enhancement in the use of
systems to distinguish valid reliable sites on health and this shows an
increase in awareness of the importance of being critical 9:47 //
some years ago it was more likely that a user of a general nature would
go on to internet and would accept everything without a critical
distance 10:1 //
and now there tends to be a more critical awareness of all of this by the users 10:8 //
this very very briefly is another American survey regarding the use of internet for health 10:17 //
the data here come from the period two thousand and two two thousand and three 10:22 //
this is a survey that was sent out to twelve thousand people 10:26 // there were about eight thousand five hundred answers 10:29 // and these were supposed to be representative of the American population 10:34 //
this survey also involved the possible presence of chronic diseases and
the type of relationship between the user and internet according to
whether they had this kind of disease or not 10:46 //
generally it was seen that thirty-five per cent of users were looking for information on health on the net 10:53 //
and they were looking in different places 10:56 //
they were looking on information sites or information portals 11:1 // which are sites which tend to guide the user towards various different sources of health information 11:9 //
then afterwards second in order of importance were government sites as I mentioned before 11:17 //
and then non profit organization sites 11:21 //
with regard to the reason for which users were looking for information
on internet there was the fact that it was easy to look for this
information 11:31 // the information was readily available 11:33 // the information was clear and simple and also free of charge 11:38 //
and then many considered that the sources were reliable 11:43 //
another thing that was emer- that emerged in this survey was that about fifty-seven per cent had one or more chronic diseases 11:55 //
this finding is an underestimation probably because in the survey there
was only a list of chronic diseases not including cystic fibrosis for
example but including depression diabetes and other diseases 12:12 //
it emerged however that among the people who indicated that they had
one or more chronic diseases among these people and people without a
chronic disease there wasnt a difference in terms of the type of site
visited according to whether they had a chronic disease or not 12:30 //
there was an increased use of services like chat rooms or online discussion for people with chronic diseases 12:40 //
from a number of studies it seems that an explanation for this may be
that people find more support or feel that they have better backing and
more support when they can communicate with other people in the same
situation and with the same disease 12:58 //
before I move on I just wanted very very briefly to show you and to
speak a little bit about quality because there are a number of
different concepts of quality 13:13 //
a number of studies have been carried out concerning the quality of sites 13:17 // but these studies didn't always assess the same kinds of quality 13:21 //
in other words some groups or some studies focus on quality assessment
vis-à-vis the compliance with certain aesthetic criteria so called
aesthetic criteria 13:35 //
for example the presence of the name of the author of the information 13:40 // the possible presence of references that's to say where the information comes from and so on 13:46 //
other studies or other groups have assessed quality of sites in terms of the completeness of the information 13:53 //
for example they chose some diseases or some subjects and they
identified certain aspects of those diseases that the site should deal
with in order to be complete 14:4 //
but with an instrument like internet its almost impossible to be able
to assess the completeness or to evaluate quality on the basis of
completeness because there are so many sites that the user will tend to
look for information on various sites 14:22 //
and so different sites are not bound to have the same kind of target usership 14:29 //
mainly there are some sites that want to give information just on one
aspect of a disease and others that want to give general information
without being too detailed in the information they give 14:40 //
and then the third concept of quality is quality in terms of guidelines or scientific knowledge 14:47 //
this is the only way in order to be able to evaluate the quality of the information on a site 14:52 //
this means that we should look at the information on the disease and on
the therapy in the site and then compare those with the state of the
art in terms of current knowledge 15:5 //
there are various strategies or initiatives that have been created in
the last few years in order to try to find a way to assess the quality
of sites and here too there are a number of different approaches 15:23 //
there are for example codes of conduct rules of conduct 15:29 //
this is one example that of the American Medical Association guidelines 15:37 //
this is a code of conduct which specifies that people who create health-related sites should comply with a number of criteria 15:45 //
then there are also systems which have striven to evaluate quality on the basis of certification by third parties 15:55 //
that's to say the so called HON code 15:57
// this meaning that another group an external group a group of third
parties will assess a site on the basis of certain criteria and will
then accredit the site giving it a certain recognition a certain
acknowledgement 16:12 //
and then there are specific guides for the users for example Discern 16:17 //
here these guides will give the user a list of criteria that the user
can apply to an individual site simply by looking focusing on a number
of questions to see whether the site is a quality site 16:37 //
there has been a larger study which has attempted to bring together all the different kinds of initiative over the years 16:48 // the finding being that there were twenty-nine different initiatives of this kind 16:53 //
there was a grouping together of the criteria and principles behind
these different initiatives and what emerged at the end of the day was
that in most cases there was a comparable situation 17:5 //
for all the strategies there was a central focus on the same kind of information 17:12 //
this now very very briefly is an example of the code of conduct I mentioned before of the American Medical Association the AMA 17:23 //
this is a code of conduct for people who create sites on health which
specifies that sites should comply with all of these criteria 17:32 //
there should be the name of the author 17:34 // there should be a reference on the site 17:37 // and there should be a declaration of the financial support received 17:43 // there should also be a certain revision of the material by experts 17:49 //
it's necessary to declare the method of revision 17:52 // it's necessary to indicate the date of when the site was created or last updated and so on 17:59 //
then with regard to the second strategy I mentioned // that's to say
certification by third parties // it's the famous HON code 18:6 // and here is the list of their guiding principles 18:9 //
that's to say that a site in order to be able to demonstrate the use
of this code in order to be able to put on the HON code label has to
comply with these criteria which are practically the same 18:25 //
that the author has to be named 18:27 // the authors credentials have to be clear 18:29 // references date of revision and so on 18:32 //
in particular this indicates that there should be the sentence that the
information provided on the site should not substitute in any way the
relationship with the doctor and the doctors contribution 18:47 //
then there's also the guide for the user //
this is the third strategy 18:51 //
again the references the name of the author responsible for the information 18:55 //
but here the guide goes one step further 19:1 //
the user who theoretically doesnt know how to evaluate the quality of the information of the site should then give an opinion 19:10 // he's invited to give an opinion 19:12 // for example regarding the specific way in which a disease is addressed 19:20 //
for example the user is asked if the site in his or her opinion will give biased information or subjective information 19:29 //
for example if it will give information which is biased or objective 19:33 //
if several sources of information are quoted 19:36 //
if there is mention of uncertain areas grey areas in treatment 19:41 // if different experts opinions are quoted 19:43
// or if the site tends to say that such and such a treatment is
efficacious in one hundred per cent of the population concerned 19:51 //
then for example in detail its necessary to look at what would happen if the disease was not treated and so on 20:2 //
all of these different criteria are the ones that are used for assessment of the sites 20:8 //
what are the problems with these various methods 20:10 //
well first and foremost all of them are linked to the fact that internet has not got a fixed structure 20:16 //
it's continually changing 20:18 //
and then furthermore as I said before there isnt an audience which is well defined 20:25 //
a site could be for a professional user 20:28 // it could be for a young child 20:31 // it could be for any end user 20:33 //
so with regard to these codes of conduct for example these codes are
not mandatory and they can be interpreted in // and they can't be //
and they may not be interpreted sometimes in the third in the same way 20:44 //
with regard to the assessment of third parties this little blue label
this little tab is applied when the site has been revised by experts
but the site may change 20:58 //
and then with regard to the guide for users this presupposes that the
user's got the time and the right mental attitude to be able to examine
the instrument correctly and the capacity to evaluate this instrument
correctly 21:13 //
there have been many many studies in order to assess the capacity of
these various strategies to measure the true quality of information on
the sites concerned 21:28 // and these studies have shown that the strategies concerned are not always applicable //
in that if we have a study which evaluates the presence of the approval
tab in the HON code its been said that the quality in some sites was
good and in other sites was not good 21:47 //
and this was irrespective of whether the site have the HON approval label or not 21:53 //
then there are single criteria with regard to the name of the author or the references and so on 21:59 //
and sites that give this information are not bound to give correct information or accurate information 22:5 //
then we must also bear in mind the level of the language used // the kind of language register used 22:13 //
this is a study of two thousand and one which assessed the information
on four diseases showing that the level of language used was a high
school level of education 22:29 // that's to say the kind of language that would be used with by a person with nine or ten years of schooling 22:34 // this is higher than in the average population 22:36 //
this regarded use of the sites by Spanish and English speaking populations 22:40 //
and with regard to a study in ninety ninety-nine this too emerged that
the level of language was a high school level of language 22:50 //
with regard to damage created by internet there has been or there have
been many warnings but there has not been actually much firm evidence 23:1 //
there has been damage in this respect and this is an ongoing situation 23:6 //
but with regard to the media message that we should be careful about
internet there isnt really much to be found in terms of firm
scientific documentation 23:19 //
there has been a review in this respect in two thousand and two with an
attempt to find all the scientific articles on possible damage and out
of one thousand five hundred publications about this subject there were
only three cases of specific damage 23:37 //
in one case it was an emotional form of damage to the person as a
result of consulting a site for information which was not appropriate
on internet 23:48 //
in one case there was damage to the dogs of a user 23:52 //
and in the third case there was a case of effective harm in a person
with carcinoma who consulted a doctor after one year of self medication
following the information obtained on internet 24:9 //
and there was also a questionnaire to UK doctors in two thousand and one 24:14 //
they indicated that among their patients there had been one case of
death from overdose of a drug which had been purchased on internet 24:25 //
and there had been two fatal cases of tumours which had been presented
late after a period of self medication on the basis of internet
indications 24:35 //
quality studies //
there have been a lot 24:38 // in all different areas and populations and all age groups 24:42 //
and these would vary from coughing and temperature fever at the Mario Negri institute 24:49 //
in part because we were asked to speak about this today 24:53 //
we have had studies also about depression and diabetes cystic fibrosis urinary incontinence and so on 25:0 //
this was reviewed and the review of all these data show that seventy
per cent of these conclusions showed that there was a poor quality of
information on internet 25:15 //
but many of the studies concerned did not assess the quality true and
proper of the information but tended to assess the aesthetic aspects of
the sites or the completeness of the sites 25:28 //
here we have a number of studies then about the quality of chronic diseases depression kidney diseases diabetes and so on 25:37 //
and in these cases the finding was that the data didnt agree 25:45 // the data were not consistent 25:47 //
some people evaluated the content on the basis of the guidelines and
the scientific knowledge and they saw in the case of diabetes for
example that the accuracy varied considerably 25:60 //
in terms of urinary incontinence it was found that the sites gave reasonably accurate information 26:6 //
whereas other studies showed that there wasnt much consistency 26:11 //
there wasn't much consistency vis-à-vis the criteria which were established by the HON code as I mentioned before 26:22 //
there is a study on the quality of information on cystic fibrosis // this was performed in two thousand and two 26:29 //
the study concerned did not assess specifically the quality but it
focused on the compliance of cystic fibrosis sites with the AMA code of
conduct 26:41 //
the finding was that there wasn't much compliance 26:46 //
less than half of the sites gave for example the data of the last update of the site 26:53 //
forty per cent gave the source or the name of the author of the information 26:58
// and still fewer sites specified that the information could not be
considered a substitute for the doctors input or the doctors care 27:9 //
so there wasnt much compliance with the guidelines 27:12 //
very briefly the results of the review that we performed recently and were still evaluating and analysing the results of this 27:20 //
we decided to perform this review on Italian cystic fibrosis sites to
see the quality and also in order to try to go one step further in
relation to the study I mentioned before on cystic fibrosis because the
sites didnt evaluate or that study didnt evaluate the quality of
information but evaluated the quality of compliance with the criteria
and also focused on English sites 27:47 //
we decided to start out with Italian sites 27:50 //
and we decided to start out in particular with Google 27:53 //
for Italian we put in the key words fibrosi cistica //
we found a hundred and seventy-five thousand pages 27:59 //
of these we limited ourselves to the pages written in Italian and we
limited ourselves to the pages where cystic fibrosis or fibrosi cistica
was part of the title of the page 28:13 //
this allowed us to focus down to narrow down the field to almost thirteen thousand pages 28:19 //
we did the same thing in English just to have a comparison in terms of scale number of pages 28:24 // here from fourteen million pages we were able to whittle things down to about four hundred thousand pages 28:31 //
I'd just like to mention briefly in this respect that by comparison
with the fourteen million pages found in April two thousand and six in
the previous study on cystic fibrosis that I mentioned before with the
same criterion only a hundred and eighty-six thousand pages had been
identified 28:51 //
this shows in the last four years a massive increase in terms of the
number of pages which can be found on health topics on the internet 29:1 //
probably however this is because of the optimization of search engines 29:8 //
so now continuing with our revision we were able to get to twelve
thousand of pages twelve thousand nine hundred pages with the title
including the words fibrosi cistica 29:20 //
we considered the first forty and we excluded the ones which didnt give enough information about cystic fibrosis 29:27 //
we were able thus to obtain fou- thirty pages which were assessable and
these were assessed by a number of persons on the basis of the
compliance with nine aesthetic criteria which had been selected by us 29:42 // and also according to the accuracy of the information about cystic fibrosis vis-à-vis the international guidelines
