so just to review what is the what are the principles of achieving better outcomes through newborn screening 0:9 //

well the idea of newborn screening for any disease is that patients are diagnosed with the disease in the preclinical stage before there are any symptoms 0:22 // so that there can be close monitoring and follow-up 0:27 //


and hopefully intervention with effect- effective therapies before around the time of the onset of symptoms in order to delay or prevent chronic lung disease and improve nutrition 0:42 //

now in cystic fibrosis the clearest benefit of newborn screening has been in the area of growth and nutrition 0:54 // it's been clearly demonstrated that newborn screening results in sustained improvement in growth 1:1 //

and this has been seen in a randomised control trial that was done in Wisconsin as well as cohort studies that were done in Australia France Italy and the Netherlands and the United Kingdom 1:15 //

here the results from the randomised control trial done in Wisconsin where patients were randomised at birth to be diagnosed either by newborn screening or conventional diagnosis 1:32 //

and you can see this is the conventional diagnosis group 1:37 // and this is the newborn screening group 1:39 //

and there are weight there are height for age disease score 1:43 // it's followed up all the way to sixteen years 1:46 // and you can see that there's a significantly higher height for age disease score 1:51 // and those identified by newborn screening all the way up to sixteen years of age compared to those who were identified conventionally 1:58 //

so sustained benefit in terms of nutritional status in those identified by newborn screening 2:4 //

interestingly results from the same study out of Wisconsin suggested there may also be important benefits of newborn screening in terms of cognitive function or IQ 2:20 //

the study showed that in patients up to two years of age there was a sig- statistically significant improvement in head circumference in those identified by newborn screening as compared to those diagnosed conventionally 2:35 //

and this cognitive score is equivalent to what we call the IQ in the United States 2:42 //


and importantly the children who were diagnosed conventionally who had low vitamin E levels at diagnosis had significantly lower intelligence quotients than those who were diagnosed by newborn screening or those who were diagnosed conventionally but did not have low vitamin E levels at diagnosis 3:6 //

and this difference on average was about eleven IQ points 3:10 //

so suggesting that there may be an important value ache- to achieve one's intelligence potential if one is diagnosed conventionally with low vitamin E levels 3:20 //

the evidence in terms of the impact of newborn screening on pulmonary disease is much more mixed than it is for nutritional outcomes 3:32 //

and there is suggestion of better lung function and or better chest x-ray scores in screened infants in cohort studies from the Netherlands Australia and France 3:48 //


but in the Wisconsin randomised control trial there was no pulmonary benefit of screening 3:55 //

so I think it remains to be seen how much influence on pulmonary outcomes there is with newborn screening over time 4:2 //

there is also evidence of decreased burden of care and reduced hospitalisations associated with newborn screening 4:12 //

in a randomised controlled trial that was performed in Britain as well as cohort studies in Australia France and a separate observational study in Britain there was decreased burden of care and decreased hospitalisations seen in those in the screened groups 4:32 //

and interestingly there have been several publications out of the United Kingdom now that have shown that the same outcomes are achieved with less burden of care in newborn screened infants compared to conventionally diagnosed infants 4:47 //

and a there was a recent publication I'm sure that you all saw in the Lancet just last month that showed that there was a huge cost savings associated with newborn screening because of this 4:59 //

what about the impact of newborn screening on survival 5:5 //

well that's been harder to show because as I told you deaths in early childhood are quite rare in cystic fibrosis 5:14 //

and so trials of newborn screening were not powered to assess survival 5:19 // however having said that there certainly does appear to be a significant impact of newborn screening on survival 5:26 //

so we evaluated the impact of newborn screening programmes on survival in the Wisconsin randomised control trial as well as in a number of European studies both in the United Kingdom randomised control trial and cohort studies out of Australia Italy and France 5:46 //

in the Wisconsin study there were no deaths in either the screened group or the controlled group up to ten years of age 5:55 //

but in all the other studies there was a higher proportion of deaths up to ten years of age in the controlled group as compared the screened group and it was statistically significant in all those groups 6:8 //

and when you pool all of the European studies there was a statistically significant increase in death in the control groups up to ten years of age as compared to the screened groups 6:19 //

so there probably is a survival benefit associated with newborn screening as well 6:24 //


lastly I'm just gonna talk a little bit about a trial that we are doing that is in its early phases and share a little bit of our preliminary results with you 6:38 //

this study is called the EPIC or Early Pseudomonas Infection Control study 6:47 //


it's funded by the United States Cystic Fibrosis Foundation 6:51 //

it's a longitudinal observational study 6:54 // it's ongoing 6:56 //

and its overall objectives are to determine the risk factors for early age and acquisition of Pseudomonas aeruginosa 7:5 //

to evaluate the relationships between Pseudomonas serology 7:10 // isolation of Pseudomonas from respiratory cultures and signs and symptoms 7:16 //

and to look at the effective antibiotic treatment strategies in young children with cystic fibrosis 7:23 //

the inclusion criteria for this study are at that the patients need to be less than twelve years of age 7:33 //

they of course have to be diagnosed with cystic fibrosis 7:36 //

and they have to be Pseudomonas aeruginosa negative 7:41 //

in other words they have to have had no prior isolation of Pseudomonas aeruginosa 7:47 //

or if Pseudomonas aeruginosa has been isolated from a respiratory culture in their lifetime it has to have not have been isolated for at least two years prior to enrolment in the study 7:58 //

within this large observational study there is a nested clinical trial called the EPIC clinical trial 8:10 // and I'll explain the clinical trial 8:13 //

there are will be three hundred subjects enrolled in the clinical trial //

so far a little over two hundred of them have been enrolled 8:21 //

and they are randomised to receive either culture-based therapy or cycle therapy 8:28 //


what does that mean 8:29 // basically patients in the in the observational study when they have Pseudomonas first isolated from a respiratory culture are eligible to enroll in this clinical trial 8:42 // if they choose to enrol then they're randomised to one of these two arms 8:46 //

cycle therapy means that they receive therapy once every quarter i e once every three months regardless of respiratory culture status 8:58 //

culture-based therapy means that they receive therapy at one moment and then further therapy only if their follow-up culture which is obtained every three months is positive for Pseudomonas aeruginosa 9:12 //

if it's negative for Pseudomonas aeruginosa they do not receive therapy 9:17 //

so the culture is obtained every three months and a treatment decision is made based on the result of that culture 9:22 //

and then the total duration of enrolment in the trial is eighteen months and then they fall back into the observational study or you provide long-term follow-up on the cohort 9:34 //

now within each one of these arm // culture-based therapy our cycle therapy // they are further randomised to exactly what kind of treatment they receive 9:49 //

so they either receive they all receive inhaled tobramicine for twenty-eight days 9:57 //

and then they are randomised to whether they also receive oral ciprofloxacin for the first fourteen days of therapy or matching oral placebo 10:6 //

and that's true in both the cycle therapy arm and the culture-based therapy arm 10:11 //

the primary study objectives are to compare the proportion of ps- of Pseudomonas aeruginosa positive respiratory cultures and the time to first pulmonary exacerbation between the cycled and culture-based groups 10:29 //

I can't share you with you any results from that study because it's not yet fully enrolled 10:34 //

and so there will be several more years so we have any results to share 10:37 //

however I can share with you some preliminary results from the observational study 10:43 //

enrolment in the observational study began in November two thousand and four and we achieved our target enrolment in October two thousand and six 10:54 //

as of April two thousand and seven we have just over seventeen hundred patients enrolled at fifty-nine centres in the United States 11:3 //

and what we've done is to evaluate the characteristics of these children at their baseline visit 11:11 //

so the data that I'm going to share with you is just from the baseline or first visit 11:16 //

the mean age at enrolment of these children was five point seven years 11:21 //

just about half of them were homozygous for the delta F five o eight mutation 11:26 //

so that's pretty reflective of the US population of people with cystic fibrosis in general 11:33 //

seventeen per cent were diagnosed by newborn screening //

I'm sure it would be much higher here in Italy 11:38 //

twenty-nine per cent were exposed to cigarette smoke 11:42 //

thirty-nine per cent less than five years of age were in daycare 11:48 //


here are the most common respiratory pathogens that were isolated from respiratory cultures at enrolment 11:58 //

about just under a half had Staphylococcus aureus 12:3 //

about sixteen per cent had Haemophilus influenza 12:6 //

about eight per cent had Pseudomonas aeruginosa 12:9 //

so that means that they came into the trial without </with_out/> Pseudomonas aeruginosa but on that first culture Pseudomonas grew 12:15 //

and then about seven per cent had meticillin resistant Staphylococcus aureus 12:21 //

these are not mutually exclusive 12:24 //

some patients had more than one of these but in general patients only had one 12:28 //

and of course some grew none of these at all 12:31 //

now we were happy to see that these children on average were very healthy 12:38 //

their average lung function was normal 12:41 //

the mean forced vital capacity was a hundred per cent of predicted 12:46 //

the mean forced expiratory value in one second was ninety-six per cent of predicted 12:51 //

they also had very nice nutritional status 12:55 //

their mean height percentile was at the thirty-sixth percentile 12:59 //

their mean weight was at the fourty-first percentile 13:3 //

and their mean body mass index was at the fifty-fou- fifty-fourth percentile 13:8 //

so these kids really on average were fairly healthy 13:13 // they also had very few symptoms as reported by the parents 13:19 //

so int- we asked the parents about cough in the previous week 13:23 //

a third of them were reported as having no cough in the previous week 13:27 //


whereas about twenty per cent were reported as having frequent or productive cough in the previous week 13:33 //

we asked the parents about activity compared to their friends or siblings 13:40 //

and about sixty per cent were reported as being as active as their friends or siblings 13:46 //

about twenty per cent as more active 13:49 //

so if you add that up that's eighty per cent who were either as active or more active than their friends or siblings 13:56 //

and only six per cent were noted to be less active 13:60 //

so it sounded like their symptoms were not impacting their day-to-day life too terribly 14:6 //

and yet having said that we found that overall they're they were receiving quite a lot of treatment 14:14 //

so just about half had received DNase chronically in the preceding three months 14:21 //

and just about half had received oral antibiotics in the preceding three months 14:27 //

and about eip- eight per cent had received inhaled antibiotics in the preceding three months 14:32 //

which is interesting because in the United States inhaled antibiotics are typically used to treat Pseudomonas aeruginosa and yet these patients were not supposed to have Pseudomonas aeruginosa 14:42 //

so it's interesting that they were receiving this antibiotic anyway 14:46 //

we also performed some prem- preliminary analysis 14:52 // these have not been adjusted yet for anything to look at the effect of cigarette smoke and neonatal screening on the characteristics of these patients at enrolment 15:2 //

and you can see some very important and striking and significant differences between the groups who were exposed to cigarette smoke and those who were not 15:12 //

so we divided the children up into those who were who whose mothers smoked after birth and those whose mothers did not smoked after birth 15:22 //

and you can see that the lung function was both s- im- clinically importantly higher and statistically significantly higher in those children whose mothers did not smoke after birth compared to those whose mothers did smoke after birth 15:42 //

and in addition height and weight percentiles were significantly higher in those children whose mothers smoked compared did not smoke compared to those whose mothers did smoke 15:54 //

so looks like cigarette smoke exposure has a very profound effect on both lung function and growth in these children 16:2 //

we did a similar analysis for a diagnosis after newborn screening 16:9 // and you can see important effects of that as well 16:12 //

so lung function was significantly higher in those diagnosed by newborn screening 16:21 // and height and weight were also significantly higher in those diagnosed by newborn screening compared to those who were not diagnosed by newborn screening 16:30 //

so it looks like newborn screening is having a very positive impact on both lung function and on nutritional status in those who are lucky enough to benefit from it 16:41 //

so in summary contributors to morbidity and mortality in cystic fibrosis are very complex and also interrelated </inter_related/> 16:54 //

we've made significant advances in our understanding of these relationships in the past decade through longitudinal cohort studies such as I've outlined here and analysis of data from various different national patient registries 17:12 //


I should point out that these epidemiologic studies cannot demonstrate cause and effect 17:18 // they are not interventional studies 17:20 //

so it's important not to infer causality from these studies 17:24 //


in terms of future directions I think that further understanding or modifier genes and of environmental exposures will be elucidated in the next five or ten years 17:39 //


and also I think that improved understanding of early CF lung disease will help our patients get started on the right track early in life and stay on it 17:49 // so that hopefully cystic fibrosis truly will someday be an adult disease 17:54 //

and I'd like to de- say end by saying we're doing something right even if we can't always predict what it is 18:3 //

quali sono i principi secondo cui si possono raggiungere migliori risultati attraverso lo screening neonatale 0:7 //

beh si potrebbe fare lo screening per qualunque malattia 0:11 // quando appunto i pazienti ricevono una diagnosi nella stas- fase preclinica di modo che ci possa essere un monitoring molto ravvicinato e anche un follow-up e anche delle terapie efficaci si spera 0:28 //

prima del periodo dell'insorgenza dei vari sintomi con un ritardo poi nella prevenzione della malattia cronica e cercando di migliorare anche la nutrizione 0:39 //


nella fibrosi cistica il vantaggio dello screening neonatale è coinvolto soprattutto l'area della nutrizione 0:52 in quanto c'è stato un miglioramento della crescita e della nutrizione 0:58 //

e questo è stato visto in uno studio randomizzato controllo del Wisconsin con studi di coorte che sono stati fatti anche in Australia in Francia in Italia nei Paesi Bassi e nel Regno Unito 1:10 //

vediamo qui i risultati di uno studio nel Wisconsin // uno studio controllo randomizzato con pazienti analizzati alla nascita a cui s- è stato diagnosticato la malattia con lo screening neonatale 1:27 //

vedete qui il gruppo convenzionale di diagnosi e questo invece è il gruppo sottoposto a screening 1:35 //

si vedono i valori fino a sedici anni 1:42 //

e si vede che c'è un livello significativamente superiore di indici qui con lo screening fino a sedici anni rispetto al gruppo che ha avuto una diagnosi tradizionale 1:55 //

quindi ci sono vantaggi sia per gli studi nutrizionali e anche per lo screening neonatale 2:1 //

è interessante vedere che altri risultati dallo stesso studio hanno dato modo di vedere che ci sono vantaggi anche sulle funzioni cognitive o sul quoziente intellettivo 2:16 //

si vede che il paziente fino a due anni di età c'era una differenza statisticamente significativa nel miglio- con risultati migliori rispetto a coloro che avevano avuto una diagnosi tradizionale invece che lo screening neonatale 2:33 //

e l'indice cognitivo si riferisce al quoziente intellettivo 2:37 // questi sono dati degli Stati Uniti 2:39 //

e si vede che ci sono i bambini che hanno avuto una diagnosi tradizionale con un livello molto più basso significativamente più basso in termini cognitivi rispetto a coloro che hanno avuto la diagnosi attraverso lo screening o coloro che avevano livelli diversi di diagnosi 3:3 //

c'erano undici punti del in- del livello intellettivo 3:7 //

quindi vediamo che è un metodo molto utile per calcolare questo potenziale cognitivo 3:15 //

abbiamo poi evidenze riguardo all'impatto dello screening neonatale sulla patologia polmonare e le evidenze qui sono molto più miste 3:27 //

si pensa che la funzionalità polmonare sia migliore o che anche un indice per la radiografia toracica nel gruppo sottoposto a screening sia vero attraverso studi di coorte che vengono dai Paesi Bassi dall'Australia e dalla Francia 3:45 //

però nello studio randomizzato controllo del Wisconsin non si riscontravano vantaggi a livello polmonare 3:51 //

quindi si deve ancora capire quanto sia effettivo questo impatto nel corso del tempo 3:58 //

ci sono anche evidenze sull'assistenza e sul l'ospedalizzazione </ospitalizzazione/> e sul peso di questo 4:8 //

in uno studio controllo randomizzato in Gran Bretagna e poi uno studio di coorte in Australia e in Francia e con osservazioni separate anche dalla Gran Bretagna si è visto che c'è stata una diminuzione delle ospedalizzazioni </ospitalizzazioni/> e del peso dell'assistenza con i gruppi sottoposti a screening 4:28 //

e poi un risultato interessante di diverse pubblicazioni nel Regno Unito che hanno mostrato gli stessi risultati con un inferiore peso dell'assistenza con neonati sottoposti a screening rispetto a coloro sottoposti a p- diagnosi tradizionale 4:46 //

nel mese scorso è stato pubblicato su Lancet uno studio anche sui costi e sui risparmi che ci sono utilizzando lo screening neonatale 4:56 //


vediamo l'impatto dello screening sulla sopravvivenza 5:1 //

come vi avevo già detto prima le morti tra neonati sono piuttosto rare ormai in fibrosi cistica 5:13 //

quindi è difficile valutare la sopravvivenza in questa fase però sembra che ci sia comunque un impatto significativo dello screening neonatale anche sulla sopravvivenza 5:23 //

abbiamo valutato qui l'impatto del programma di screening neonatale sulla sopravvivenza nello studio controllo randomizzato del Wisconsin e anche in altri studi europei 5:36 // il Regno Unito con un controllo studio randomizzato </ran_domizzato/> 5:39 // uno studio coorte australiano e anche dell'Italia e della Francia 5:43 //

nello studio del Wisconsin non c'erano decessi nel gruppo sottoposto a screening o nel gruppo controllo fino ai dieci anni di età 5:51 //

però in tutti gli altri studi si sono registrati valori più alti a dieci anni di età di decessi nel gruppo controllo rispetto al gruppo sottoposto a screening 6:1 // e queste differenze sogno significativamente sono statisticamente significative 6:6 //

vedete che questo è vero in tutti gli studi europei con un aumento dei decessi nel gruppo controllo a dieci anni di età rispetto al gruppo sottoposto a screening 6:15 //


quindi sembra che ci siano vantaggi sulla sopravvivenza anche nello screening neonatale tra pazienti più giovani 6:22 //

infine vorrei parlare di uno studio che è tuttora in corso che stiamo conducendo 6:29 // siamo nelle fasi iniziali 6:32 // stiamo adesso lavorando su dei risultati preliminari 6:35 //

questo studio si chiama EPIC che sta per studio controllo sull'infezione precoce da pseudomonas 6:44 //

è stato è in corso nella Fondazione fibrosi cistica degli Stati Uniti 6:51 //

è uno studio coorte longitudinale in corso 6:54 //

e si vuole determinare i fattori di rischio per l'età precoce di acquisizione della pseudomonas aeruginosa 7:1 //

per studiare la relazione fra la sierologia della pseudomonas gli isolati della pseudomonas dalle colture respiratorie e i segni e i sintomi 7:12 //

e vogliamo anche guarda- vedere gli effetti delle cure antibiotiche nei giovani pazienti con fibrosi cistica 7:19 //

il criterio di inclusione per questo studio comprende l'età dei pazienti che deve essere inferiore ai dodici anni 7:29 //

deve essere stato diagnosticato con fibrosi cistica 7:33 //

e non ci dovevano essere altri isolati di pseudomonas aeruginosa //

non no dovevano essere negativi precedentemente alla pseudomonas 7:43 //


oppure se avevano avuto un isolato di pseudomonas aeruginosa doveva avere una storia inferiore ai due anni precedente ai due anni per poter essere incluso nello studio 7:57 //


in questo studio questo studio in pratica proviene da uno studio clinico 8:5 // è il risultato di un altro studio clinico uno studio osservazionale 8:9 //

nello studio clinico ci saranno trecento soggetti coinvolti 8:15 //

finora duecento sono stati coinvolti 8:17 //

nello studio randomizzato avremo sia terapie basate su colture oppure terapie cicliche 8:24 //


in pratica i pazienti nello studio osservazionale quando hanno av- avuto un primo isolamento di pseudomonas sono stati poi coinvolti in questo studio clinico 8:38 //


con la terapia ciclica significa che ricevevano la terapia una volta ogni tre mesi al di là degli studi di coltura respiratoria 8:55 //

invece la terapia basata sulla coltura voleva dire che ricevevano la terapia ogni volta o ulteriore terapia solo quando si ottenevano delle colture positive ogni tre mol- tre mesi alla pseudomonas aeruginosa 9:8 //

se la coltura era negativa non avrebbero ricevuto alcun tipo di trattamento 9:13 //

quindi si aspettavano altri tre mesi prima di prendere un'altra decisione sulla base dei risultati di tale coltura 9:19 //

il tempo di coinvolgimento è diciotto mesi per poi tornare nello studio osservazionale 9:27 //


all'interno di questi gruppi tra terapia basata sulla coltura oppure ciclica abbiamo fatto due gruppi randomizzati per vedere esattamente quello che ricevevano 9:46 //

tutti ricevevano la tobramicina per via inalatoria per ventotto giorni 9:53 //

e poi i randomizzati ricevevano anche la ciprofloxacina orale per i primi quattordici giorni di terapia oppure un placebo orale 10:2 //

e questo è stato applicato in entrambi i casi di terapia ciclica o basata sulla coltura 10:8 //

gli obiettivi principali dello studio sono stati di confrontare la proporzione di colture positive alla pseudomonas respiratorie e poi quelle con la prima esacerbazione polmonare tra i gruppi trattati ciclicamente oppure sulla base della coltura 10:27 //

come dicevo lo studio è in corso //


abbiamo bisogno ancora di tempo per poter darvi dei risultati 10:34 //

però abbiamo dei risultati preliminari dallo studio osservazionale 10:40 //


il coinvolgimento è cominciato a novembre del duemila e quattro dei pazienti con un reclutamento target raggiunto poi nel duemila e sei 10:51 //

e ad aprile duemila e sette avevamo ottenuto millesettecentodue pazienti coinvolgendoli in cinquantanove centri degli Stati Uniti 10:59 //

quello che abbiamo fatto è stato una valutazione delle caratteristiche dei bambini nella prima visita 11:7 //

volevamo vedere com'erano i pazienti alla prima visita 11:13 //

l'età media era di cinque virgola sette anni 11:16 //

circa la metà erano omozigoti per la mutazione delta F cinquecento otto 11:23 //

quindi questo dimostra come la popolazione statunitense sia rispetto alla fibrosi cistica 11:30 //

il quarantasette una buona percentuale il diciassette per cento era stata diagnosticata con lo screening neonatale 11:36 //


il ventinove per cento era stato esposto a fumo di sigaretta 11:38 //

e il trentanove per cento aveva passato meno di cinque anni in assistenza giornaliera 11:46 //

poi ci sono i patogeni respiratori che sono stati isolati da coltura 11:54 //


al momento abbiamo circa la metà con lo stafilococco aureo 11:58 //

circa il sedici per cento aveva l'emofilus influenzae 12:1 //

e il sette virgola otto per cento aveva pseudomonas aeruginosa 12:4 //

e questo significa che sono venuti a far parte dello studio s- con questa percentuale di pseudomonas aeruginosa 12:11 //

e poi circa il sette per cento aveva lo stafilococco aureo resistente alla meticillina 12:17 //

questi non sono non si escludono l'uno con l'altro questi dati //

alcuni potevano essere misti ma in genere i pazienti ne avevano solo uno 12:25 //



siamo stati lieti di vedere che in generale questi bambini erano piuttosto sani 12:35 //

la loro funzionalità polmonare era normale in media 12:38 //

la FVC media era centouno per cento come previsione 12:44 //

la FEV uno era del novantasei per cento come previsione 12:47 //


avevano anche uno status nutrizionale normale 12:51 //

il percentile di peso med- di altezza media era trentasei 12:55 //

il peso medio era come percentile quarantuno 12:59 //

e l'indice di massa corporea il valore medio era cinquantaquattro come percentile 13:5 //

quindi in gene- in media questi giovani pazienti erano piuttosto sani e pochi erano i sintomi che si presentavano 13:14 //

abbiamo chiesto ai genitori riguardo alla tosse nelle precedenti settimane 13:20 //

alcuni hanno dichiarato che non c'erano stati segnali di tosse nelle settimane precedenti 13:26 //

o il diciannove per cento aveva dichiarato che c'era stata una tosse frequente o produttiva 13:30 //

poi abbiamo anche analizzato l'attività rispetto agli altri giovani della stessa età 13:37 //

il sessantuno per cento di questi giovani era attivo allo stesso modo 13:43 //


il ventuno per cento era stato valutato più attivo 13:45 //

quindi l'ottanta per cento era o più attivo o attivo al pari dei loro compagni 13:51 //


e il sei per cento soltanto risultava meno attivo 13:55 //

quindi sembrava che i sintomi non avessero un impatto troppo negativo sulla loro vita 14:2 //

ma pur dicendo questo avevano comunque dovevano comunque sottoporsi a diversi trattamenti 14:11 //

circa la metà aveva ricevuto DNase a livello cronico nei tre mesi precedenti 14:17 //

e quasi la metà aveva ricevuto antibiotici per via orale nei precedenti tre mesi 14:23 //

e l'otto per cento aveva ricevuto antibaio- antibiotici per via inalatoria nei tre mesi precedenti 14:28 //

che il che è interessante perché negli Stati Uniti gli antibiotici per via inalatoria si usano soprattutto per la pseudomonas aeruginosa e questi pazienti non dovevano avere la pseudomonas aeruginosa 14:39 //

quindi ricevono comunque ricevevano comunque l'antibiotico per via inalatoria 14:44 //

abbiamo anche svolto alcune analisi preliminari che non sono ancora però pronte per risultati riguardo al fumo di sigaretta con lo screening neonatale 14:55 // e le caratteristiche dei pazienti quando sono stati coinvolti 14:59 //

si vede che sono dati molto importanti // si vedono differenze notevoli fra i gruppi tra il gruppo esposto e il gruppo non esposto al fumo di sigaretta 15:9 //

abbiamo diviso i bambini a seconda delle madri se fumavano dopo la nascita oppure no 15:19 //

vedete che la funzionalità polmonare era più alta a livello clinico e a livello statistico nel gruppo dei bambini le cui madri non hanno fumato dopo la nascita 15:36 // invece il valore è inferiore nel caso in cui le madri fumassero 15:39 //

e anche il percentile dell'altezza e del peso era significativamente superiore per quei bambini le cui madri non fumavano rispetto ai bambini le cui madri fumavano 15:52 //


quindi l'esposizione al fumo di sigaretta ha un effetto davvero forte sulla funzionalità polmonare e sulla crescita dei pazienti giovani 15:60 //

abbiamo fatto anche diverse analisi con lo screening neonatale rispetto allo screening neonatale e si vedono anche qui differenze importanti 16:11 //

vedete che la funzionalità polmonare era significativamente superiore con i pazienti sottoposti a screening neonatale e anche l'altezza e il peso era significativamente superiore rispetto al gruppo che non era stato sottoposto a screening neonatale 16:25 //


quindi sembra davvero che lo screening abbia un impatto positivo sia sulla funzionalità polmonare sia sullo status nutrizionale ovviamente per coloro che sono abbastanza fortunati per poter </potrer/> trarne vantaggio 16:39 //

quindi riassumendo i contributi </contri_buti/> sulla morbilità e la mortalità nella fibrosi cistica sono difficili //

i fattori che contribuiscono a questi sono difficili e sono intercorrelati 16:50 // abbiamo fatto passi significativi in avanti nel capire le relazioni tra questi fattori negli ultimi anni attraverso studi di coorte longitudinali come quelli che vi ho mostrato qui e anche dall'analisi di dati presenti in diversi registri nazionali di pazienti 17:9 //

questi studi epidemiologici non possono dare s- un contributo di causa-effetto ovviamente 17:16 //

quindi è importante non trarre questo tipo di conclusioni dai risultati che vengono proposti 17:21 //

riguardo alle direzioni future penso che una comprensione ulteriore dei geni modificatori e delle esposizioni ambientali verranno messe spiegate chiaramente nei prossimi anni e aiuteranno a capire anche il la patologia polmonare precoce 17:42 //

nel capire questo aiuterà i pazienti a immettersi nel percorso giusto e fare sì che la fibrosi cistica sia una patologia dell'età adulta 17:53 //


vorrei concludere dicendo che stiamo facendo qualcosa di giusto anche se non sappiamo possiamo sempre prevedere cos'è che stiamo facendo 18:0 //
